Before marking a task done:

• Objective from task body is met
• All acceptance criteria satisfied
• Outcome report written to docs/kanban-outcomes/<task-id>-outcome.md
• All artifacts committed or staged
• kanban_complete called with summary and metadata
• No secrets/tokens in summary or metadata
• Child tasks created if follow-up work identified
